PRELIM. SKILLS EVALUATION

Instructions:

1. Download a blank preliminary swim form here.
2. For each Nipper, complete the form and practical and have it signed off by a qualified person (refer form).
3. Enter the name/s of each Nipper included on the form/s below.
4. Scan or photograph the form/s, save to a file and upload below. One completed form per Nipper.

Alternatively, form can be handed to your Age Manager of the first day of the program.

Please Note: It is your responsibility to ensure forms are provided prior to the program commencing. No Assessment = No Nippers.
